Right of access request form

This form allows you to exercise your right of access under Organic Law No. 63 dated 27 July 2004 on the protection of personal data (GDPR equivalent).

To enable us to respond as quickly as possible, we require proof of your identity. Please attach a copy of a valid identity document (national identity card (CIN), passport, or residence permit).

If the data subject is a minor, please also attach a copy of the valid identity document of at least one parent. Est considéré comme mineur par la Loi, toute personne concernée ayant moins de 18 ans.
If the data subject is under guardianship or curatorship, or has appointed an agent to exercise their rights, please also attach a copy of the valid identity document of the guardian, curator, or agent, as well as the power of attorney.

Reference: Organic law number 63 dated July 27, 2004 relating to the protection of personal data, article Art. 40. “The person concerned, their heirs or their guardian, may request to rectify the personal data concerning them, complete them, modify them, clarify them, update them, delete them when they prove to be inaccurate, incomplete , or ambiguous, or request their destruction when their collection or use has been carried out in violation of this law.
